To make a nomination you must do three things:

  1. Read the Nomination Announcement which will give you all the info you
    need on eligibility and fees
  2. Fill out the online form below. You must finish the form to click submit, which means you must have wording of the citation. No cheating this year!
  3. For your nomination package to be complete you must send or email the nominee’s resume to Lisa Rother.
